About the Job Job Summary: MedStar Health is looking for a Patient Navigator/Community Health Educator to join our team! Responsible for coordination and facilitating access to health-related services for clients by utilizing available resources. Primary Duties Supports client care coordination by conducting needs assessments, identifying eligible individuals, and facilitating access to appropriate health services and resources. Maintains accurate and timely documentation in EMR and tracking systems to monitor client interactions, services provided, and outcomes. Promotes health education, self-management, and quality improvement initiatives through client engagement, counseling, and support. Collaborates with internal teams, external partners, and community agencies to ensure continuity of care and address care gaps. Ensures compliance with organizational policies, safety standards, and regulatory requirements while actively participating in trainings, meetings, and grant-related activities. Qualifications High School Diploma or GED with 2 years of experience (Required) Bachelor’s degree (Required) Master’s degree in public health or related field (Preferred) 1-2 years in a healthcare environment. Strong understanding of cultural competency with target population.
